A medication error is defined as “any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the health care professional, patient, or consumer” according to the National Coordinating Council for Medication Error Reporting and Prevention.*  Medication errors can occur in both hospital and outpatient settings.

Medication Error Causes and Medical Malpractice

There are many causes of medication errors and all are preventable. Drug name confusion, poor communication, misinterpreted or sloppy handwriting, improper drug labeling or instructions, poor medical knowledge, a lack of protocol or process, or failure to properly educate a patient can all result in fatal or harmful medication errors. Examples of medication errors include:

  • Prescribing, administering, or dispensing the wrong medication to a patient.
  • Failure to prescribe a needed medication in a timely manner.
  • Prescribing, administering, or dispensing the wrong dosage of a medication. Either too much, or too little of a medication can cause harm to the patient.
  • Incorrect dosing instructions, for example, the patient is instructed to take a medication too often, or too infrequently.
  • Administering a medication by the wrong route. For example, an oral drug given intravenously, or vice versa.
  • Administering a medication that is known to cause a harmful effect when given with another drug that has been prescribed for the patient. This harmful effect is referred to as a drug-drug interaction.
  • Prescribing a drug that is contraindicated for a particular patient.
  • Prescribing a drug to a patient with a known drug allergy.
